Jockey Club "Stand-by U" Caregivers Community Support Project

Aims

  • To develop a screening-guided in-home respite care service model

  • To build an integrated online platform, including needs assessment, volunteer training assessment, volunteer matching, case management, and video-based respite

  • To evaluate the effectiveness and cost-effectiveness of the service model

  • To consolidate best practices of the screening-guided in-home respite service model

Beneficiaries

  • Family Caregivers taking care of homebound older adults

  • Homebound older adults living in the community who have care needs

  • Trained volunteers and substitute caregivers who provide in-home respite services

Scope

  • Five NGO community partners covering Kowloon, Hong Kong Island and parts of the New Territories.

  • Caregivers will be segmented into 4 levels of risk/needs assessed by proprietary multi-dimensional screening tool and be triaged to the relevant services accordingly.

  • On-line platform will be created to maximise efficiency of service operation.

Significance and Impact

​In-home respite service project addresses the challenges caregivers face in providing homebound elderly care by:

  • Creating a responsive and effective screening-guided in-home respite care model.

  • Empowering caregivers, enhancing their well-being, and improving the lives of the elderly.

  • Filling gaps in existing support services, creating a caregiver-supportive network and benefiting Hong Kong’s aging population.

Project Duration

The proposed project will last for 3.5 years, starting from April 2024.​
